lossyFLAC is the combination of lossyWAV and FLAC. LossyWAV removes some less important information that doesn't affect the signal at standard settings, offering larger compression ratios. You can get a better understanding of the process here.

Older encodes will have the cover art embedded. Newer releases will include external cover art as well as embedded for compatibility.
